What do I need to get a birth certificate?

You can obtain a birth certificate by providing the following:
- Full (maiden) name
- Date of Birth
- Parents' Full Names (mother's maiden name)
- Copy of picture ID (ie: driver's license)
- Check/money order payable to City Clerk
